Complete Your Intake
Download, print, and complete the applicable forms. Mail or fax them at least one week before your first appointment.
Step03
Prepare for Your Visit
Arrive at least 15 minutes early. Bring your insurance card, prior evaluations, and pertinent medical or school records.

Begin Your Care Journey
Attend your scheduled appointment with the appropriate provider.
