    Frequently Asked Questions page

We are credentialed providers with VI Equicare, and are able to accept most insurance plans, including Blue Cross Blue Shield, Cigna, and United Healthcare. If your insurance is not accepted, we can provide you with a billing statement to submit to your insurance company. At this time, we do not accept M.A.P., but are happy to work with your family on a sliding scale basis. The patient will be responsible for any costs not covered by insurance.

    Know your financial responsibilities

Please confirm your individual plan’s benefits, referrals, prior authorization, deductibles, co-insurance, co-payments, non-covered services, and provider participation with both your insurer and the office. Being insured does not guarantee coverage for a particular service.

The practice’s original financial policy requires co-payments and applicable patient responsibilities at the time of service. Patients are responsible for remaining balances after insurance processes a claim. Review the complete policy and ask the billing department about current terms and payment arrangements.
